Our clinic is dedicated to offering compassionate, comprehensive care to patients of all ages. We are currently welcoming new patients and provide access to experienced family doctors serving the Markham community. From preventative care to ongoing health management, our team focuses on ensuring every patient feels valued and supported. Conveniently located, Swan Lake Medical Clinic aims to be a trusted partner in your family’s health.

Patients/Parents are responsible for accurate and up to date health card information.

Please bring your health card for each visit. Failure to provide accurate and up to date health card information will result in your claim to OHIP being rejected. Rejected claims are subject to $20.00 administrative fees plus the cost of visit.

Patients will be charged $50 for a no-show appointment. Please be considerate and attend your appointment or cancel at least 12 hours in advance so that other patients can access the doctor and avoid any no-show penalties.

Please notify our staff of changes to your mailing address, email address or telephone number.

For emergency care, please call 911 to arrange transport to your nearest emergency department.

You are eligible to enrol in the family practice if you have OHIP and if you are not already rostered to another family health practice. You must also live within 100km the clinic in case a physician needs to see you in person. To Get Started Click Here!

Swan Lake Medical Clinic aims to keep wait times as short as possible. While we work to see patients efficiently, wait times may vary depending on patient volume and clinical needs.

Yes. Once you visit the doctor and complete your consultation, if you receive a prescription, take it to Pharmasave Swan Lake Pharmacy friendly and knowledgeable pharmacy team.

Swan Lake Medical Clinic is currently welcoming new patients. Availability for family doctor attachment may vary based on physician capacity. Patients are encouraged to speak with our clinic team to learn more about the registration process.
